Overview of Court Forms
The probate court numbering system for forms is based on jurisdictional and functional categories and is designed to facilitate the identification and location of appropriate forms. The prefix “PC” denotes “Probate Court." As approved by the Procedures Review Committee of the Connecticut Probate Assembly, each jurisdictional category will correspond to a one thousand series of numbers, and within each jurisdictional category, numbers will be assigned by 100s, according to their function in the proceeding and the order in which they would normally be used.

3000 Series: Conservatorship
4000 Series: Fiduciary
5000 Series: Guardianship of Minors
6000 Series: Termination of Parental Rights and Adoption
7000 Series: Persons with Intellectual Disability
8000 Series: Mental Health Commitments
9000 Series: Miscellaneous
