All Probate Court forms available on the website were updated on May 7, 2015 to include a QR code. Members of the public with a smartphone may scan the QR code to access our website. Where applicable, the code will link directly to information on the type of probate matter associated with the form. 
 
The addition of the QR code also represents a significant advancement in the Probate Court’s file management system. When the document containing the QR code is scanned, the court’s computer system will automatically detect, identify and populate the type of form, and determine whether the form is confidential and recordable. This capability will enable courts, and users, to more easily retrieve particular documents in a court file and on the public access computers.
 
Given the importance of the new QR code for both the public and the courts, we ask that attorneys and the public only use forms that have been downloaded from our website after May 7, 2015.
